At MetroRite Clinic, we define a household as individuals living together who share common living arrangements and expenses. The size of your household plays a key role in determining your eligibility for the Sliding Fee Discount Program. Whether you are a single individual or part of a larger family unit, we take into account the number of people in your household.
Annual Income Assessment
In addition to household size, we also consider your annual income when evaluating your eligibility for the Sliding Fee Discount Program. Income includes: earnings, unemployment compensation, worker's compensation, social Security, Supplemental Security income, public assistance, Veteran's payments, survivor benefits, pension or retirement income, interest, dividends, rents, royalties, income from estates, trusts, educational assistance, alimony, child support, assistance from outside the house hold and other miscellaneous sources.
Application Process
To apply for the Sliding Fee Discount Program, simply contact our clinic and our staff will guide you through the application process. Our Sliding Fee Discount Program offers uninsured and under insured patients discounts based on annual income and household size. The program follows Federal Income Guidelines to determine patient eligibility. To remain eligible for the program, you must complete the Sliding Fee Discount Application every 12 months or if your financial situation changes.
